Focusing efforts only on today's segment of the population that has already has multiple chronic diseases begs the question of how to prevent other people from becoming tomorrow's segment of the population that will need to have the their illnesses addressed. The typical medical approach proposed here that only focuses on illness once is has occurred needs to shift to a more proactive mode. The medical home offers some promise of helping make that shift...if done right.
